Request to issue a separate Passport to Child

When you are making a passport for your child for the first time, you must submit a Letter of Consent/No Objection to the Department of Immigration and Emigration.

Form 1 – "Request for issuance of aseparate passport for a child" is the form you need to fill out

Important

  1. If both Father and Mother live in outside of the Sri Lanka, they both should fill and sign one form and get signature from the Embassy of Sri Lanka
    1. The passport will besent from the Embassy to the Immigration Department
  2. If one of the parents lives in Poland and the other is in Sri Lanka, the person living in Poland should fill out and sign the form and get it signed by the embassy. The form submitted to the embassy, ​​after being signed, will be returned to the relevant parent. It can be forwarded to the parent in Sri Lanka and the child's passport can be prepared
  3. You can forward the scanned copy of this duly filled form to theembassy's email address and avail the relevant service: slemb.warsaw@mfa.gov.lk or consular@srilankaembassy.com.pl
  4. After signing the form, a scanned copy of the form will be emailed to you
