New Passport for a Baby

  1. Duly filled application‍
  2. Recently taken Color photograph – must be properly dressed (Passport size, white back ground, clear visual ears) 03 Nos.
  3. Scanned/Soft copy of Photo (JPG format) – When you take photos at the studio, remember to get a soft copy of the photo on your phone or email.You can also email the soft copy of the photo to the embassy: (slemb.warsaw@mfa.gov.lk  or consular@srilankaembassy.com.pl)‍
  4. Original Birth Certificate
  5. Citizenship Certificate with a copy of the applicant is a citizen by registration (If the child was born inPoland or another country)
  6. Concern letter from parents (Please refer Form1 – “Request to issue a separate Passport to Child”
  7. Copies of Passports of parents
  8. Copies of TRC of Poland of parents
  9. Passport charge;
    1. For10 years PLN 595 (without any bank charges)
    2. For3 Years PLN 485 (without any bank charges))
