Documents required for driver's license renewal

  1. Duly filled application (Please send us your address with postal code to send the application (slemb.warsaw@mfa.gov.lk  or consular@srilankaembassy.com.pl)
  2. Current driving license
  3. OriginalBirth Certificate – TAKEN AFTER 2008 (If you do not have an original certificate, you can get one through the embassy. It only costs 100 zlotys.)
  4. MedicalCertificate (from Poland) with English translation (must)
  5. Recently taken Black and White photograph – must be properly dressed (Passport size, clear visual ears) 02 Nos
  6. Current Passport
  7. Copy of the National Identity Card
  8. Copyof Visa card in Poland (TRC) - If you do not have aPolish visa or TRC card, a confirmation document issued by the relevant authority to confirm that a visa has been applied for is sufficient for the process
  9. DrivingLicense renewal fee USD 115 (without any bank charges
  10. Late fee per year USD 10 (without any bank charges) - - If your Driving License is expired for any category by even one day, you will be charged a fine of US$10 per year.

Note:

  1. If you are coming to the Embassy to get your Driving License processed, there is no need to pay cash to the bank. You can pay cash to the office on the day of your arrival (Cash payments only. Card payment facilities are not available)
  2. There is no photo studio near the embassy. Therefore, always make sure to bring photos to avoid delays.
